CLOSURE OF TWO SACRAMENTO COUNTY HEALTH CENTERS - US Fed News Service, Including US State News

SACRAMENTO, Calif., Feb. 27 -- Sacramento County issued the following press release:

Three centers remain open

Today is the last day of services for two Sacramento County health clinic sites. The Capital Health Center, 1500 C Street, Sacramento, and the Northeast Health Center, 7805 Auburn Boulevard, Citrus Heights, will close their doors tonight at 5 p.m.

Three sites will continue to serve Sacramento County CMISP patients including:

1) Primary Care Center (includes chronic disease management and public health services) 4600 Broadway, Sacramento

Monday through Friday, 8 a.m. to 9 p.m.

2) Del Paso Health Center

Monday through Friday 8 a.m. to 5 p.m.

3) South City Health Center (includes chronic disease management)

Monday through Friday 8 a.m. to 5 p.m.

Those who currently use Capital Health Center and Northeast Health Center may use any of the three remaining health centers. Some of them require an appointment. Those eligible for services should contact the nearest health center for further information and to schedule an appointment.

To apply for CMISP coverage, one must:

1) Have a current medical need

2) Must be referred for eligibility determination by a medical or dental clinic run by the Sacramento County Department of Health and Human Services, or an emergency room that contracts for outpatient services under the CMISP program, or a hospital that contracts for inpatient services under the CMISP program.
